• Two sets of attested photo copies of all the educational documents (Matric, FSc. MBBS, PHOC Experience), testimonials, experience certificate, PHDC/PMC experience certificate, domicile two photographs, CNIC and valid registration from PMDC/PHC be submitted and must reach the office of the undersigned.

• The journals) should be duly indexed by PMC/ PMDC/ PubMed/Index Medicus/Excerpta Medica/ Embrase.
• PHOC/PHC registration valid on the closing date for recept of applications will be accepted.
• If expired, applying proof for renewal on or before the closing date is required.
• The candidate must possess a domicile certificate of any districts of the Province of Punjab and a copy of the same must be submitted with the application published while
• PMC/DC and FOS or PS or FRCS or MRCP or MD or Diplomat of American Board (Gener Surgery or Medicine, whichever is specialty) or event medical qualification from an ite or Three (3) years aching or practical experience in the se or after post-graduation

• Those who have been removed dismissed from government service cannot apply
